60 minute New Client Intake Session – $125
50 minute Counseling Session – $100
80 minute EMDR Session – $140
60 minute Individual or Small Group Yoga Session – $90
15-45 minute Individual Mindfulness Coaching – $25 to $75

Nevada Medicaid Fee for Service (FFS)

Administrative Fees

Medical record and other documentation – $25 per request up to first 50 pages; .25 per page for each page in excess of 50 pages. Clinical Summaries and Treatment plans are available in your Client Portal for no charge.
Legal requests including deposition preparation, court attendance, documentation, travel and other legal requests – $100 per hour. A $300 retainer is due in advance.

Medical and legal requests require 3 business days to process. An additional Express Request fee of $200 will be added to requests required in less than 3 business days.

Payment

Payments are required at the time of service. I accept cash, check and all major credit cards as forms of payment.

Cancellation Policy

If you are unable to attend a session, please make sure you cancel at least 24 hours beforehand. Otherwise, you will be charged a $25 fee.
